We actively assist in enrolling patients in the insurance marketplace.
The Affordable Care Act (ACA) created health insurance affordability programs to help individuals and families access and pay for medical insurance coverage as well as eliminate barriers to coverage. Individuals and families, including people with special health care needs, pre-existing conditions, low-income childless adults, seniors, young adults, and self-employed individuals have a path to health care insurance.
To reduce the number of uninsured individuals in our communities, the Care Program actively assists in enrolling patients into the insurance marketplace.
AIDS Assistance Drug Program (ADAP)
You may also fill out your application for the AIDS Assistance Drug Program (ADAP) online. Please email your filled-out application to CM@careprogram.care (Adobe Acrobat Reader DC required)
Please include the following documents, if applicable:
Proof of income dated within the past 90 days:
- Pay stubs (2-4)
- Bank statements
- SSA benefit letter, etc.
Most recent federal tax return:
- All 1040 forms
- 1095A
- Form 8962
- All ‘Schedule’ forms
Insurance cards (health, dental, etc.)
Monthly billing statement(s) from your insurance company
The Ryan White Grant
The Ryan White Grant is the “payer of last resort.” This means that, if a patient with HIV/AIDS has insurance, we must use it. In addition, this also means that patients with HIV/AIDS cannot be on the Ryan White Sliding Fee Scale if they are eligible for another benefit that would pay for their healthcare at Howard Brown (like Medicaid). Patients must have proof of their HIV/AIDS diagnosis on file at Howard Brown to qualify for the Ryan White Sliding Fee Scale.
If you’re having trouble getting or affording your medications, we can help ensure that your medical needs are met. We also have a pharmacist for consultation purposes.
